Tinnitus, Aural Fullness, and Hearing Loss in a Patient with Acoustic Neuroma and Pituitary Macroadenoma

We report the case of a 51-year-old woman with multiple otologic and vestibular symptoms. She presented two different types tinnitus in her right ear, vertigo, fluctuating aural symptoms left ear. also complained disequilibrium; chronic headache; hyperhidrosis; amenorrhea; insomnia; broadened hands feet; widened, thickened, stubby fingers. The patient underwent careful collection medical history, otomiscroscopy, pure tone audiometry, tympanometry, reflex threshold measurements, assessments, blood tests, magnetic resonance imaging (MRI), cone beam computed tomography (CBTC) head. audiogram showed: (1) mild low-to-mid frequency conductive hearing loss, sharply sloping sensorineural loss above 4000 Hz ear; (2) low-frequency MRI 3D FLAIR sequences detected an acoustic neuroma (7.4 mm × 5.2 mm) middle-third internal auditory canal, pituitary macroadenoma (13 10 saccular hydrops. CBCT scan documented outbreak otosclerosis (3 around fissula ante fenestram Therefore, (right ear), growth hormone-secreting gland, Menière’s disease (left ear) were diagnosed/strongly suspected. A watch-and-wait strategy was adopted for otosclerosis, while transsphenoidal surgery successfully performed to remove macroadenoma. This confirms that disorders can occur simultaneously same patient, requiring prompt audiological evaluations.
